Send a Slack alert when an admin changes a subscriber’s tier and notifies them

Fires only when the admin enables “Send notification” at the time of the tier change — not on every tier change. Use to supplement the built-in tier_change email with a Slack alert, CRM event, or secondary channel. Fires for both single and bulk tier changes.
